2014 Non-Medicare Prescription Drug Formulary

The drug formulary is a list of FDA-approved prescription drugs and supplies developed by ESI. MCHCP's formulary is based on generic drugs. The formulary is updated on a semiannual basis, but it can change throughout the year.

Generic drugs, approved by the FDA, are proven to provide the same reliable, effective treatment as brand-name versions, but at lower prices. If a generic drug is not available, talk to your doctor about taking a lower-cost brand-name drug on the formulary.

This formulary applies to benefits provided under MCHCP's Prescription Drug Plan. Retirees who have primary coverage through Medicare use the Express Scripts Medicare Prescription Drug Plan, and have a separate formulary.

For additional information on the formulary, contact ESI.
