Read the proof of eligibility requirements To find information about the documentation MCHCP requires for dependent coverage. Proof of Eligibility
Enroll in or waive coverage through the Statewide Employee Benefit Enrollment System (SEBES) To complete benefit enrollment within 31 days of your hire date. Your coverage is effective the first of the month after your hire date and enrollment through SEBES.

(If you are a recent hire and have not received a temporary password for access to SEBES, contact your Human Resource or Payroll representative immediately.)
Complete Strive for Wellness® activities To complete the Partnership Agreement and take the Health Assessment within 60 days of coverage effective date or by May 31, 2014, whichever is earlier, to receive the Partnership Incentive; and submit a tobacco attestation during your enrollment to receive the Tobacco-Free Incentive.

If you are covering a spouse, he or she must complete a tobacco attestation within 31 days of your enrollment.
Pay your first month's premium Double deductions may be taken from your payroll to pay for your first month's coverage depending on your enrollment date. Subsequent premiums are payroll deducted. Failure to Pay Premiums
Visit myMCHCP To view personal benefit information, update contact information, complete the Partnership Agreement and Health Assessment, complete a tobacco attestation, access the Health Care Provider form, and make changes to your coverage during Open Enrollment. myMCHCP
