|Read the proof of eligibility requirements||To find information about the documentation MCHCP requires for dependent coverage.||Proof of Eligibility|
|Enroll in or waive coverage through the Statewide Employee Benefit Enrollment System (SEBES)||To complete benefit enrollment within 31 days of your hire date. Your coverage is effective the first of the month after your hire date and enrollment through SEBES.
(If you are a recent hire and have not received a temporary password for access to SEBES, contact your Human Resource or Payroll representative immediately.)
|Complete Strive for Wellness activities||To complete the Partnership Agreement and take the Health Assessment within 60 days of coverage effective date or by May 31, 2014, whichever is earlier, to receive the Partnership Incentive; and submit a tobacco attestation during your enrollment to receive the Tobacco-Free Incentive.
If you are covering a spouse, he or she must complete a tobacco attestation within 31 days of your enrollment.
|Pay your first month's premium||Double deductions may be taken from your payroll to pay for your first month's coverage depending on your enrollment date. Subsequent premiums are payroll deducted.||Failure to Pay Premiums|
|Visit myMCHCP||To view personal benefit information, update contact information, complete the Partnership Agreement and Health Assessment, complete a tobacco attestation, access the Health Care Provider form, and make changes to your coverage during Open Enrollment.||myMCHCP|
