ID Cards for State Members

Use the following table to determine whether you will receive a new ID card for the 2010 plan year. Also note that all new medical plan cards will reflect a $0 copayment for preventive services and wellness exams. To learn more about your MCHCPid, visit the MCHCPid section.

Cardholder Tips:

  • Take your medical plan card to doctor’s visits
  • Take your dental plan card to dentist’s visits
  • Take your Express Scripts, Inc. card to fill a prescription
  • If you need another ID card or your card is lost or stolen, contact the plan to request one
  • Keep your cards in a safe place
  • Don’t allow people not covered under your plan to use your ID card to obtain services

If you didn’t receive an ID card or you lost it, follow these directions:

UMR
Choose one of the following options:

  • Call UMR’s customer service department at 888-200-1167
  • Call UMR’s automated system at 800-320-3206
  • Log in to your myMCHCP account at www.mchcp.org
    1. Select UMR: Access your medical benefit and claim information button
    2. Select myTools tab
    3. Select ID Card Order
    4. Select Submit button

Mercy Health Plans (State Members in South Central and Southwest regions only)
Choose one of the following options:

  • Call Mercy Health Plans customer service department at 800-450-3349 or
    417-820-8071
  • Go to www.mercyhealthplans.com
    1. Select Login or Register under Member Connection on far right
    2. Select Benefits
    3. Select Request ID card
    4. Fill out form and select Submit

Express-Scripts
Call Express-Script’s customer service department at 800-797-5754

Delta Dental
Choose one of the following options:

  • Call Delta Dental’s customer service department at 866-737-9802
  • Go to www.deltadentalmo.com
    • Select Subscribers
    • Select My Benefits
    • Select View General Benefits
    • Log in using your Social Security number, last name and date of birth
    • Select Request an ID Card
  • To receive a temporary Delta Dental card:
    1. Go to www.deltadentalmo.com
    2. Select Subscribers
    3. Select My Benefits
    4. Select View General Benefits
    5. Log in using your Social Security number, last name and date of birth
    6. Select View Benefits
    7. Select Print an ID Card
