Care Allies Smart Steps® Program

disease management imageIf you live with a chronic health condition, you may be a candidate for disease management. MCHCP offers a disease management program through Care Allies Smart Steps®. This program is for active employees, non-Medicare eligible subscribers and their dependents who are covered under an MCHCP medical plan. Mercy Health Plans in the south central and southwest regions has its own disease management programs which are not available through CareAllies. Smart Steps® is not available to members enrolled in Mercy Health Plans in these regions.

The Smart Steps® program is designed for members with chronic health conditions and offers free and confidential support for the following conditions:

  • Diabetes
  • Coronary artery disease
  • Congestive heart failure
  • Asthma

If you have any of these conditions, Smart Steps® will provide help, support and education depending on your needs. This program is free to all participants.

The Smart Steps® program does not replace the care you currently receive from your physician for treatment of your chronic condition. This program translates your doctor's treatment plan into daily actions. A personalized step-by-step plan will help you to lead a healthier lifestyle and prevent serious complications commonly associated with your chronic condition.

All information including health condition and identity is kept in strictest confidence and is not shared with MCHCP.

As part of your benefit, MCHCP includes Smart Steps®, a disease management program through CareAllies. This program can help prevent recurring symptoms and maintain a higher quality of life. Mercy Health Plans in the south central and southwest regions has its own disease management programs which are not available through CareAllies.
