Q. If I am not the subscriber,
how can I find out if my child is covered?
A. You are required to provide MCHCP proof that you are
the parent of the minor child. Then information is provided
to you, unless there is an order on file restricting your
access to the records.
Q. How can I obtain information
from MCHCP on my spouse’s coverage?
A. You can obtain information on your spouse’s coverage
if your spouse provides MCHCP with a signed authorization
to release information to you.
Q. If I request information on
my elderly mother who is covered through MCHCP, I’m
told I have to provide a signed authorization from her before
MCHCP’s customer service representatives provide information
to me. Why?
A. If you are requesting specific information that is considered
to be protected health information (PHI), such as plan name,
claim information, premium, etc., MCHCP requires the authorization
in order to protect the privacy of our members. The authorization
forms are required in an effort to keep your personal information
private.
Q. Why can I obtain information
on my 17 year old covered child but not on my 18 year old?
A. When your child reaches the age of 18, although he/she
maintains coverage through you, he/she is legally an adult.
Therefore, MCHCP must have his/her consent prior to releasing
any protected health information (PHI) to his/her parent(s).
Q. Can MCHCP assist me with claims
problems?
A. YES, MCHCP representatives are available to assist you
with many situations, including claims problems. However,
you need to provide your MCHCPid and PIN prior to assistance
being given. If you want assistance with claims for a dependent
age 18 or older or your spouse, that person must submit a
signed authorization to MCHCP before we can assist you.
Q. Why do I have to answer so
many questions before the MCHCP representative will assist
me?
A. If you provide the representative with your MCHCPid and
PIN, you do not have to answer a series of questions. If you
do not have that information, you are required to answer a
series of questions to verify your identity. This is done
to maintain confidentiality and protect your personal information.
If calling on behalf of a dependent age 18 or older, a signed
authorization must be submitted to MCHCP prior to assistance
being given.
