Missouri Consolidated Health Care Plan invites you to participate in Lifestyle Ladder, the state's wellness program administered by StayWell Health Management.

Hello, my name is Tina. I am here to tell you about climbing toward a good State of health using Lifestyle Ladder.

All non-Medicare subscribers and their spouses covered by an MCHCP medical plan are eligible to participate in the program. The purpose of Lifestyle Ladder is to educate members about health and longevity while providing tools and support to make healthy lifestyle decisions. Prior to open enrollment, the Lifestyle Ladder brochure is mailed to the homes of eligible subscribers. Participation in the program, including information in your Health Assessment, or HA, is completely confidential.

Participation in the program begins with the online completion of the HA. The HA is a questionnaire regarding your health and your lifestyle habits. The HA is completed online by logging onto myMCHCP and following the links to Lifestyle Ladder. After your HA is submitted, your overall health is assessed and health risks are identified. You have access to online programs and health care professionals who provide coaching and support.

Participation in Lifestyle Ladder includes engaging in programs either online, by phone, or by paper to earn points. You must earn 100 points per quarter to receive the incentive the following quarter. The incentive is a $15 reduction in premium for an individual and a $30 reduction for subscriber and spouse. Participation for you and your spouse in the program are tracked independently.

Points are kept in the StayWell Online Point$ Bank, which can be monitored online or by calling StayWell. You can also view a full list of activities and your point balance by logging in to myMCHCP and following the link to StayWell Online (SWOL).

If you earn more than 100 points in a given quarter, the additional points will roll over to the next quarter.

If you or your spouse discontinue participation or do not earn 100 points in a quarter, your monthly premium will increase either $15 or $30 for the following quarter. You may re-engage at any time and if you meet the point requirement, will qualify for the incentive premium the following quarter.

Employees may take the HA at any time during the year. New employees and/or newly added spouses may also take the HA during the year. You would then be eligible to qualify for the incentive premium the following quarter.

As a participant, you will receive access to online healthy living programs, articles, videos, calculators, healthy challenges and campaigns. It is your responsibility to meet the point requirements each quarter.
