My name is Amanda, and I'm going to talk about the enrollment and eligibility guidelines for the Missouri Consolidated Health Care Plan.
When you were first hired, you had the opportunity to enroll yourself and any dependents into the benefit options. Legal spouses are eligible to be covered on your plan. Dependent children are eligible up to age 25 if they are not married and are not emancipated.
You may only enroll during the year due to a life event. To enroll, complete the M-2 form, the Change/Cancellation form. The form is available on this website and in your Enrollment Guide. The M-2 should be submitted directly to MCHCP by mail or fax.
Life events include death, birth, placement for adoption, marriage, or a loss of eligibility under a different plan.
If you or your spouse has a baby, you may enroll the newborn as well as any other eligible dependents within 31 days of the date of birth. In addition, a child placed for adoption may be enrolled within the first 31 days. Even if you, the employee, are not covered; you may enroll yourself and the newborn or child placed for adoption.
Coverage is effective the first day of the month of birth or the first of the month in which an adopted child is placed.
Coverage on other newly added dependents is effective the first day of the month following or coinciding with the birth or placement for adoption and receipt date of the form.
In addition, you may add a new spouse within 31 days from the date of marriage. Coverage on your new spouse and any dependents added is effective the first day of the month following or coinciding with the date of marriage and receipt of the form.
When enrolling dependents, Proof of Eligibility such as a marriage license and/or birth certificate is required. A complete list of Proof of Eligibility requirements can be found in your Member Handbook.
Another life event which allows you to enroll yourself or dependents is a Loss of Eligibility. This means coverage for you, your spouse or your dependents is lost because either you or your dependents are no longer eligible to continue in the current plan. For example, your spouse has resigned, retired or been laid off from his or her job resulting in a loss of insurance. Or, if you or your children lose Medicaid status, that is a Loss of Eligibility which allows the affected individuals to enroll. Only those losing coverage can enroll.
Supporting documentation indicating who lost coverage, when coverage ends and why the coverage ended is required. You have 60 days from the date of loss to enroll and coverage is effective the first day of the month following or coinciding with the receipt of the M-2 form.
Each year at Open Enrollment, during the month of October, you have an opportunity to make changes to your coverage. Changes made during this time are effective January 1st of the following year.
