Use these provider links to locate participating hospitals and physicians in your plan.

- Select Click here to access the First Health Provider Search Application
- Select from one of the available options
- Select a Product, choose First Health Network
- Members in All Other Regions - UnitedHealthcare Choice Plus
- Select Medical or Behavioral Health
- Additional Network Providers in Trenton & Chillicothe areas
- Members in Northeast Region - First Health
- Mercy Health Plans HMO (Southwest and South Central Regions Only)
- Select Click to begin your search under Provider Directory in the center of the page
- Select Doctor, Hospital, Urgent Care, or Facilities
- Select State of Residence, choose your state
- Coverage Type, choose Employer
- Employer Plan or Plan Type, select HMO or POS plan
- Express Scripts
- Must be a Registered Member on ESI site
- Log in with your account information
- Select Find a Pharmacy under Understanding your Benefits
- Delta Dental - You are taken to the appropriate plan directory
- Vision Service Plan
- If you are a registered member, enter your User ID: and Password: under Please Log In
- Select Log In
- If you are not a registered member, you may:
- Choose to continue as Not a member?
- Select Continue as a Non Member
- Or, Register now to access as a member
- Select Last four of SSN and complete the form to register
- Choose to continue as Not a member?
- Magellan Health Services
- Must be a Registered Member on Magellan's site
- Log in with your account information
- Select Search for a Provider by Zip Code (must enter a zip code) or by Name
