Enroll in or waive coverage through the Statewide Benefit Enrollment System (SEBES) To complete benefit enrollment within 31 days of your hire date.

(If you are a recent hire and have not received a temporary password for access to SEBES, contact your Human Resource or Payroll representative immediately.)

Pay your first month's premium Double deductions may be taken from your payroll to pay for your first month's coverage depending on your enrollment date. Subsequent premiums are payroll deducted. Failure to Pay Premiums
Visit myMCHCP To view personal benefit information, update demographics, complete the Health Assessment (HA), and make changes to your coverage during Open Enrollment. myMCHCP

Event Information Helpful Links
New employee coverage date Your coverage becomes effective the first of the month coinciding with or following your eligibility date and the date your enrollment is received by MCHCP. Your eligibility date is the first of the month following your hire date and the earliest date that MCHCP coverage can begin.

Example: Your eligibility date is June 1 and you completed your enrollment on June 20. Your effective date is July 1. If enrollment is received by MCHCP on May 31, your coverage goes into effect on June 1 or (if requested) July 1.
Eligibility and Effective Dates
Take the Health Assessment Available one month after MCHCP receives enrollment information Take the online Health Assessment (HA) through myMCHCP to qualify for the incentive premium. You and/or your spouse have until December 25, 2009 or March 25, June 25 or September 25, 2010 to take the HA in order to receive the incentive premium for the following quarter. No other points for program participation will accrue until you take the HA. myMCHCP
Lifestyle Ladder
Annual Open Enrollment Open Enrollment occurs October 1-31 each year for coverage effective the following January.

Open Enrollment allows you to:
  • Add or cancel coverage for you and/or your dependents
  • Change medical plans or elect medical, dental and vision coverage
