Participating
as a Retiree
- To participate as a retiree,
apply for coverage prior to your retirement but no
later than 31 days after
the date of your retirement. This also applies to currently
enrolled terminated-vested subscribers who wish to continue
coverage into retirement. If you are enrolled in a plan
where you work as an active employee, when you retire, you
must change to a plan that is available where you live.
Coverage is effective on the date of your retirement. There
can be no lapse in coverage.
- To apply for coverage, complete the Retiree
Health Insurance Election (M-5) form. If you enroll
under provisions of having other insurance coverage for
six months, you must attach proof of the other coverage
including effective dates of coverage and termination date
to the Retiree
Health Insurance Election (M-5) form. Return the completed
form and applicable attachments directly to MCHCP.
- As a retired member, you receive a contribution towards
your premium. The remainder of the premium is deducted from
your retirement check. If your retirement check is not large
enough to cover the premium, you are billed each month.
- If you participate in the Cafeteria Plan the year of
your retirement and have your premium deducted pre-tax,
you can request all payroll reductions remaining at the
time of retirement be deducted from your payroll. The request
must be listed on your Retiree
Health Insurance Election (M-5) form and submitted at
least 30 days prior to your retirement.
