Disease Management
Q. What is Smart Steps®?
A. Smart Steps® is a disease management program offered by
MCHCP in conjunction with CareAllies. It is designed to help members
with chronic conditions maintain a higher quality of life.
Q. Who can participate in the program?
A. Smart Steps® is available to eligible employees, non-Medicare
retirees, terminated vested, long-term disability and survivor subscribers
and their non-Medicare spouses who qualify. Children can participate
in the asthma and diabetes programs.
Q. Do I have to participate in the disease management program?
A. Smart Steps® is an opt out program. You are placed in the
disease management program based on your claims experience, however,
you are enrolled until you choose to opt out.
Q. How do I qualify for the disease management program?
A. You are placed in the disease management program based on your
claims experience. You must have one of the following chronic conditions
to qualify for the program:
- Asthma.
- Coronary artery disease.
- Congestive heart failure.
- Diabetes qualify for the program.
Q. What do I get for participating
in the disease management program?
A. If you take the online PHA, you will receive a lower health
insurance premium or incentive rate, free and confidential support,
and maintain a higher quality of life.
Q. What do I have to do to get the incentive rate?
A. You must complete the online
PHA and participate in either Lifestyle Ladder or Smart Steps®.
If you discontinue participation during the year, your premium will
increase.
Q. When do I complete the PHA?
A. If you are an eligible subscriber, you must complete
the online PHA during Open Enrollment to receive the incentive
rate.
Q. How do I complete the PHA?
A. Log on using your PIN and
MCHCPid or Social Security number and follow the prompts.
Q. Will my employer see the answers to my PHA?
A. NO, absolutely not. Neither will MCHCP. The program is totally
confidential.
