Eligibility Requirements
You may enroll
in one of MCHCP’s plans if you are an active State of
Missouri employee in a position covered by the Missouri State
Employees’ Retirement System (MOSERS)
and are not currently covered under another retirement or
benefit plan supported by State contributions, unless the
agency sponsoring the other retirement or benefit plan joins
MCHCP. You may also enroll if you are a member of the Public
School Retirement System (PSRS)
and employed by a State agency. If you are a part-time employee,
check with your human resource/payroll representative to determine
if you are eligible for MCHCP participation.
To enroll yourself and/or your dependents when first eligible,
submit a completed Change/Cancellation
(M-2) form to MCHCP. You must provide Social Security
numbers for yourself and your dependents when you enroll with
MCHCP in addition to proof of eligibility
for your dependents.
A person who fails to meet the eligibility requirements specified
shall not be eligible to enroll or continue enrollment with
MCHCP. A person whose coverage with MCHCP was terminated due
to a violation of a material provision of the Plan (such as
fraud) shall not be eligible to enroll with the Plan for coverage.
