Effective
Date
The effective date of your coverage is the first of the month
coinciding with or following your eligibility date and the
date your enrollment is received by MCHCP. If your enrollment
is received by MCHCP prior to your eligibility date, you may
elect coverage on your eligibility date or the first of the
month following that date. Specify on the Change/Cancellation
(M-2) form the date you want coverage to begin.
Example: Your
eligibility date is June 1 and you completed your enrollment
on June 20. Your effective date is July 1. If your enrollment
is received by MCHCP on May 31, your coverage goes into effect
on June 1 or (if requested) July 1.
Your effective date of coverage
cannot be prior to the date of receipt of your enrollment
by MCHCP.
Payment for your first month’s premium must be made
by check or regular payroll deduction. Double deductions may
be taken to pay for your first month’s coverage depending
upon the date your enrollment is received and the date you
specify as your effective date. Subsequent premium payments
are deducted from your payroll.
If you do not elect coverage during your eligibility period,
you are considered a late
entrant.
