Become a MCHCP Contracted Producer

Requirements for Agency to Become Contracted

If your agency wishes to become contracted with MCHCP, the following items must be submitted for each producer in your agency that will be placing coverage with MCHCP

1. Agency/Brokerage Confidential Data Sheet
2. Individual Confidential Data Sheet
3. Current Missouri Producer License and evidence of Errors and Omissions coverage

Requirements for Individual Producers to Become Contracted

If your agency does not wish to be contracted with MCHCP, then each individual producer wishing to become a contractor must submit the following items:

1. Individual Confidential Data Sheet
2. Current Missouri Producer License and evidence of Errors and Omissions coverage

Completed forms may be mailed or faxed.  

MCHCP Review Process

• MCHCP reviews completed forms and documents
• MCHCP mails contract to producer to be signed and returned.
• MCHCP arranges for a brief training program designed to acquaint producers with MCHCP and our offerings.
• Upon completion of all requirements, producer will be authorized to place coverage through MCHCP.
