Dental Plan - through Delta Dental

There are two options with Delta Dental, the Basic and High options. Employers may offer only one option.

The $50 individual annual deductible can be met under any category or combined categories.

Basic Option

High Option

Diagnostic and Preventive - paid at 100% with no deductible.
Basic and Restorative - paid at 80% after $50 annual deductible.
Major Services - paid at 50% after $50 annual deductible.
Orthodontic Care - paid at 50% after $50 annual deductible. (For dependent children under the age of 19.)
