Terminating
Coverage
You
may terminate medical coverage at any time during the year.
The termination takes place at the end of the month in which
the form is received by MCHCP. However, if you participate
in a pre-tax savings plan (Cafeteria plan), changes that affect
your deduction must be approved by them. Contact your human
resource/payroll representative for additional information.
If you terminate coverage, you cannot enroll in an MCHCP plan
until the next Open Enrollment.
Special Notes Concerning Dependent
Coverage: You cannot terminate coverage on your spouse
or children of the marriage during
divorce or legal separation proceedings. If you drop
dependent coverage due to a divorce or legal separation, note
this information on your form and supply a copy of the divorce
decree. MCHCP is required to notify these dependents regarding
eligibility for COBRA coverage. Therefore, you must advise
MCHCP of the current address of the affected dependents.
You may NOT terminate dental and/or vision
coverage on yourself or your dependents during the year unless
you also terminate medical coverage, terminate employment
or retire. |
