Use your PIN and MCHCPid numbers when contacting our office or accessing myMCHCP.
You receive an MCHCPid card when you become eligible for benefits, and forms are
received in our office.
Active Employees - Return all completed forms to your
human resources/payroll office.
Other Members - Return all completed forms to MCHCP.
