Instructions
Open Enrollment
Active Employees:
Open Enrollment is the annual period when you may enroll
in medical and/or optional plans and change your level of coverage.
If a different plan is available, you may enroll, change plans or
add or cancel dependent coverage.
Retired, terminated vested or survivor
subscribers: Open Enrollment is the annual period when you
may choose to continue coverage, change plans (when a different
plan is available) and drop dependents from coverage. However, you
CANNOT add dependent(s). If you cancel coverage on yourself or your
dependents, you cannot enroll at a later date.
All subscribers must enroll in the plan available where the Public
Entity is located. If there is more than one office location, the
plan is based on where the main office is located.
After the Open Enrollment period is over, if you need to make a
change in your plan selection and another plan is available, you
must appeal to the MCHCP Board of Trustees by the end of January.
After January 31, NO APPEALS FOR PLAN CHANGES can be made.
MCHCPid and PIN
Use your personal identification number (PIN) with your MCHCPid
or Social Security number to enroll during Open Enrollment using
the web or phone.
Facts About Your MCHCPid and PIN
- Your MCHCPid remains the same each year. It cannot be changed.
- You can change your PIN at any time using the web or phone
enrollment system.
- Your PIN and MCHCPid cannot be given over the phone. If you
lost or forgot them, you can request MCHCP mail them to your home.
Terminating Coverage
Medical - You may terminate
coverage on yourself and/or your dependents anytime during the year.
However, if you participate in a pre-tax savings plan (Cafeteria
plan), changes that affect your deductions must be approved by them.
Dental/Vision -
You can terminate dental and/or vision plan(s) during Open
Enrollment for coverage to end December 31, 2006. Once enrolled
in the dental and/or vision plan(s), you cannot terminate coverage
during the year on yourself and/or your dependents, unless you also
terminate medical coverage, terminate employment or you retire.
Transition
of Care
If you change to a different medical plan for 2007 and:
- Have a serious medical condition,
- Are currently under case management, or
- Have ongoing medical treatment or care,
MCHCP wants to ensure the transition goes smoothly. In order to
access ongoing care or treatment for yourself or a family member
with the least amount of disruption, contact your new plan for 2007.

- Upcoming Surgery
- Prospective Transplant
- Maternity Care
- Home Nursing Care
- Radiation Therapy
- Dialysis
- Durable Medical Equipment
- Clinical Cancer Trials
- Physical/Speech/Occupational Therapy
- Chemotherapy
- Oxygen Therapy
- Hospice Care
