Change
of Address
Check with your human resources or payroll office for the Enroll/Cancel/Waive/Change
form to complete when making changes.
MCHCP maintains address information for the health plans, so it is imperative that
your address be correct in order for you to receive ID cards, explanation of benefits
(EOBs), etc. It is your responsibility to keep MCHCP informed of your current address.
Address changes can be made:
- Online using myMCHCP.
- In writing.
- By calling MCHCP.
