Thank you for viewing this presentation today about the vision plan.
Hello, my name is Lisa.
The vision insurance is available through Vision Service Plan or VSP and is separate from your medical plan. VSP has providers nationwide and does not issue ID cards. To access services, you simply make an appointment with a VSP network optometrist or ophthalmologist and tell them you are a VSP member.
An annual eye exam is covered with a $10 copayment. You may obtain lenses for your glasses OR contact lenses annually. If you choose lenses for your glasses, there is a $25 copayment which includes frames every other year.
There is a $120 retail allowance for frames. If you choose frames that cost more than $120, you will receive a 20% discount on the difference. For example, if you choose frames that are $150, there will be a 20% discount on the $30 difference.
If you prefer to receive contact lenses, VSP covers up to $125 for the contacts annually. If a fitting fee is charged, the fee is applied to the $125 allowance.
In addition, VSP offers a "value added benefit" for extra pairs of glasses purchased during the year. If you buy an extra pair of glasses or sunglasses from a VSP provider, you will be given a 20% discount.
Laser procedures are covered at a discounted rate at specific VSP providers. Contact VSP to learn which providers are available and the discounted amount.
For additional information, including non-network reimbursement amounts, refer to your member handbook. Monthly premiums are found in your enrollment guide.
To contact VSP, call 800-877-7195 or visit their Web site at www.vsp.com. Or contact MCHCP at 800-487-0771.
