Dental Plan Claim Pre-determination
If the care you need costs less than $200 or is emergency care, have your dentist proceed with treatment. If it costs $200 or more and is not emergency care, you could ask your dentist to submit a treatment plan to Delta Dental of Missouri and receive a pre-determination of benefits before you proceed with treatment. The pre-determination of benefits provides you with an estimate of costs and enables you to know, in advance, the amount to be reimbursed by Delta Dental and how much is your responsibility.
